Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b). Observed bleach gel on shelf above sink in art room, blue chemical in spray bottle on shelf (windex?) and Lysol spray on shelf in Maths class. Keep all hazardous items stored according to label and away from students.

Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed minor stained ceiling tile on hallway leading into gym. EHS advises school to ensure no active leak is occurring and to have stained ceiling tile replaced.

Observed water stained ceiling tiles in hall leading to Gym and in front of display area in hall leading to Media center. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed aerosol Lysol spray on desk in room 2L and green disinfectant spray (unlabeled) on shelving in room 2F. Keep all hazardous items stored away from children. -Repeat: Observed stained ceiling tiles in the gym, and classes. Repair the issue and replace the tiles. -Observed tissue wads on the ceiling in both gym restrooms. Clean. -Observed cracked floor tiles in the boy's gym restroom. Repair. -Observed damage under the handsink (in the cabinet) in room 2C. Repair. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

Observed boxes of paper towels stacked on the floor in the custodial/storage closet. Follow the rule below. All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.

Observed water damaged ceiling tiles in last classroom(right side) and in hallway near gym. Observed peeling paint on window near gym. Observed cracked floor tiles in boys room in gym. Repeat. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.

-Observed loose toilet seat in one of the hall girl's restroom, and a broken toilet seat in the boy's gym restrooms.-Observed a loose faucet in the boy's hall restroom.-Observed one toilet with no water in it in the girl's hall restroom.Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).

Observed pink buildup on a few water fountains.Drinking fountains shall be kept clean and in good repair. 15A NCAC 18A .2406 (b).

-Observed cans of wasp spray in 1N, disinfectant wipes in 1A, and disinfectant sprays in 1B. All items were stored unlocked and within the reach of the children.Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).-GENERAL COMMENT: Observed a pool with standing water for watering the garden. Collecting water in this manner allows for mosquito breeding. Discontinue process.There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a).

Observed the water pressure was very low at a drinking water fountain in the upper grades hallway.**Drinking fountains shall be provided with adequate water pressure and be properly regulated (i.e. water pressure should be regulated so that an individual’s mouth does not come in contact with the nozzle and so that the water does not splash on the floor). 15A NCAC 18A 2406 (b).

Observed some toilet seats were loose, and the urinal stall partitions with chipping paint.**Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).

Observed the HVAC vents in the most restrooms were dusty.**All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).
